An Envoy Airlines Embraer ERJ-145, registration N625AE performing flight MQ-3033 from Chicago O'Hare,IL to Springfield,MO (USA) with 53 people on board, was being vectored for approach to Springfield's runway 02 at 4000 feet MSL when a large bird impacted the aircraft. The crew did not observe any anomalies or adverse handling of the aircraft and continued for a landing without further incident. There were no injuries, the aircraft received substantial damage.

On Nov 20th 2015 the NTSB released their brief final report concluding the probable cause of the accident was:

an in-flight collision with a bird

The NTSB reported that a post flight inspection revealed substantial damage to the radome, right wing, right horizontal stabilizer and right hand engine (AE3007).
