Shandong B738 at Jinan on Jul 30th 2023, temporary runway excursion on landing

A Shandong Airlines Boeing 737-800, registration B-1273 performing flight SC-8856 from Nanchang to Jinan (China), performed an ILS approach to Jinan's runway 19 in low visibility, winds varying between 116 to 156 degrees at 12.5 to 20.5 knots, with the captain as pilot flying manually flying the aircraft with a Head Up Display (HUD). The aircraft was entirely stable on approach until the first officer announced "runway in sight" at 100 feet AGL at 150.5 KIAS. At that point a left rudder pedal input began with a right bank angle of nearly 3 degrees, the speed began to decrease to 142 KIAS until the first officer called "speed", thrust was increased, the aircraft accelerated to 149 KIAS, the left rudder input increased further, the aircraft rolled to the left at 2 degrees left bank. The first officer called for a go around, however, the call was not heard by both the captain and the second first officer on the observer seat. The CDI increased from 0.02 dots to 0.192 dots until touch down about 500 meters past the runway threshold and about 9 meters to the left of the runway center line at 179 degrees magnetic 7 degrees off the runway heading at 1.554G at 139 KIAS at 20:29L (12:29Z). The crew applied full rudder to the right, however, 2 seconds after touchdown the aircraft went past the left hand edge of the runway about 600 meters past the runway threshold, the left hand main gear went off the paved surface of the runway about 645 meters past the runway threshold, travelled on soft ground for about 220 meters before returning onto the paved surface of the runway. The aircraft slowed safely without further incident and taxied to the apron, where the crew advised ATC they might have contacted runway edge lights at the left hand side. 4 edge lights on the left hand side of the runway were subsequently found damaged.

China's CAAC released their final report in Chinese only (Editorial note: to serve the purpose of global prevention of the repeat of causes leading to an occurrence an additional timely release of all occurrence reports in the only world spanning aviation language English would be necessary, a Chinese only release does not achieve this purpose as set by ICAO annex 13 and just forces many aviators to waste much more time and effort each in trying to understand the circumstances leading to the occurrence. Aviators operating internationally are required to read/speak English besides their local language, investigators need to be able to read/write/speak English to communicate with their counterparts all around the globe).

The report concludes the probable causes of the serious incident were:

- The crew's insufficient control of the risk of operating in heavy rain at night

- the crew's insufficient control of the aircraft in heavy rain and night time, inadequate preparation for approach and landing, and inadequate implementation of Standard Operating Procedures.

- After the aircraft crossed the decision height the captain mainly relied on the HUD display, lost the HUD display at 30 feet AGL but did not initiate a go around.

- Below 30 feet AGL the captain's vision did not fully adjust and he did not have a clear understanding of the aircraft status which led to development of 5.4 degrees left bank and 7 degrees difference of trajectory to runway heading resulting in touch down at 1.55G 9 meters to the left of the runway center line.

- Although the captain corrected with full right rudder immediately after touch down, he was unable to stop the aircraft from veering left off the runway.

- The operator's flight training, technical management, scheduling management, crew dispatch and policy dissemination.

- The captain's visual acuity test (tested after the incident) did not meet the relevant CCAR-67 standards.
