A Skippers Aviation de Havilland Dash 8-300, registration VH-XKI performing a charter flight from Meekatharra,WA to Leinster,WA (Australia)with 49 passengers and 4 crew, was in the initial climb out of Meekatharra when the crew detected abnormal engine indications and shut the right hand engine (PW123) down. The aircraft returned to Meekatharra for a safe landing.

Australia's TSB rated the occurrence an incident and opened an investigation.

On Jul 27th 2017 the ATSB released their final report without formal conclusion reporting that the aircraft was climbing through about 1000 feet AGL the aircraft experienced a vibration throughout the airframe and the aircraft noise changed its pitch. The crew noticed the right hand propeller was turning at 500 fpm (normal governing range 900-1200 rpm) with the engine torque being excessively high. The crew concluded they were dealing with an unscheduled autofeather of the right hand propeller, shut the engine down, declared PAN, verified the runway at Meekatharra was suitable for landing back and landed back.

The ATSB reported:

The operator’s maintenance organisation downloaded the flight data recorder for analysis by the engine manufacturer. It was determined that an uptrim signal was sent to the left engine by the right engine TSCU and the right engine experienced an over-torque event of 146% for 25 seconds.7 Both engine control units provided several fault codes, which were investigated with no defects found. The right engine TSCU and propeller hub and blades were replaced in accordance with the directions from the respective manufacturers. The right engine reduction gearbox8 and oil system were inspected with no defects found. In addition, the right engine electronic controller was replaced for troubleshooting purposes.
