A BH Air Balkan Holidays Airbus A319-100 on behalf of Spicejet, registration LZ-AOA performing flight SG-152 from Mumbai to Delhi (India) with 152 passengers and 6 crew, was climbing through 4000 feet out of Mumbai when the crew received a "HYD G ENG 1 PUMP LO PR" ECAM message. While working through the ECAM actions the crew also received "HYD G SYS LO PR", "F/CTRL FLAPS FAULT", "HYD Y RSVR OVHT", "HYD G + Y LO PR" messages with associated ECAM actions. Faced with the failure of two hydraulic systems (green and yellow) the crew declared emergency and decided to return to Mumbai. The flight attendants were briefed on the pending return and made according announcements to the passengers, who in return started to put on their life jackets, a number of them inflated the life jackets despite announcements to not inflate the life jackets. The captain took control of the aircraft, the first officer relinquished control, assumed the role as pilot monitoring and calculated the landing performance data. The captain performed an alternate gear extension. While descending through 1000 feet AGL on final approach to Mumbai's runway the captain turned the yellow hydraulic pump on in a "last minute attempt" to achieve better landing performance, the yellow system recovered. The aircraft landed safely on Mumbai's runway 27 32 minutes after departure and was towed to the apron.

India's DGCA released their final report concluding the probable cause of the incident was:

Failure of the Green Hydraulic System due loss of hydraulic fluid from the damaged 'LH MLG door actuator flexible hose' caused emergency landing of the aircraft. Failure of Yellow Hydraulic System could not be confirmed on ground.

The DGCA analysed that the flexible hose of the left hand main landing gear door was found pierced near its union thread and leaking fluid. Loss of fluid and loss of pressure/system of the green hydraulic system were attributed to that damaged hose. The damaged flexible hose was replaced. The overheat of the yellow hydraulic system could not be confirmed on the ground. The pilot decision to return was justified, the attempt to recover the yellow hydraulic system was successful. The pilots' handling of the aircraft did not contribute to the occurrence.

The DGCA analysed that weather did not contribute to the occurrence.

The DGCA analysed that cabin crew made announcements to the passengers, including in Hindi, to put on their life jackets, when a number inflated their life jackets announcements were made not to inflate the life jackets and a number of passengers were prevented to inflate their life jackets. The handling of the occurrence by cabin crew was satisfactory.
