A PIA Pakistan International Airlines Boeing 777-200, registration AP-BGZ performing flight PK-782 (scheduled dep Dec 5th, actual dep Dec 7th) from Toronto,ON (Canada) to Karachi (Pakistan), was climbing out of Toronto when the crew stopped the climb at FL250 due to the cabin not properly pressurizing. The aircraft returned to Toronto for a safe landing on runway 05 about 40 minutes after departure.

The aircraft had returned to Toronto on Dec 5th following an emergency descent due to the loss of cabin pressure as result of a bleed air fault with the pressure regulating shut off valve malfunctioning. The aircraft had received maintenance at the Air Canada Hangars, but suffered the same problem again on the next departure. The occurrence aircraft finally made it to Karachi arriving on Dec 10th with a delay of 92 hours (3 days and 20 hours).
