An Expressjet Embraer ERJ-145 on behalf of United, registration N14143 performing flight EV-4580/UA-4580 from Milwaukee,MI to Newark,NJ (USA), departed Milwaukee's runway 07R when passengers observed a right main tyre burst during the departure roll with debris impacting and damaging the right hand wing. The crew obviously remained unaware, contacted departure using routine communication and climbed from 5000 to 13000 feet when the crew indicated to ATC they needed to return to Milwaukee due to a technical problem subsequently advising an engine (Ae3007) appeared to be using more oil than normal. In the meantime another departure went without incident and without pilot report from runway 07R. The aircraft landed safely back on runway 07R about 20 minutes after departure and vacated the runway. Following the landing a runway inspection was carried out.

A passenger reported during the departure roll a right hand tyre burst, debris impacted and punctured the wing at the leading edge, debris also went through the right hand engine which emitted continuous light sparks for the remainder of the flight. A fellow passenger got out of his seat and rushed to the purser about 60 seconds after becoming airborne to tell her about the tyre, the purser subsequently called the flight deck.
