Air Canada A319 near Winnipeg on May 30th 2012, two hydraulic systems failed after leak

An Air Canada Airbus A319-100, registration C-FZUG performing flight AC-119 from Toronto,ON to Calgary,AB (Canada) with 123 people on board, had just reached the top of climb at FL360 when the yellow hydraulic system indicated a low fluid quantity. About 20 minutes later, in the vicinity of Sault Ste Marie,ON, the crew noticed the power transfer unit (PTU) was operating. The crew ran the checklists and switched the PTU off, the PTU however continued to run until its circuit breakers were pulled. In the vicinity of Winnipeg the crew noticed fluctuations in the green hydraulic system's pressure and "bumps" in the movements of the flight controls. The crew decided to divert to Winnipeg. While on approach to runway 36 the crew selected flaps 2, the flaps however did not extend beyond position 1 and the green hydraulic system showed overtemperature. The crew aborted the approach, declared emergency and performed an alternate gear extension. The aircraft landed safely on runway 36 at a higher than normal speed abotu 30 minutes after aborting the first approach and came to a stop after about 7200 feet/2200 meters with hot brakes causing the outboard right tyre to deflate. The brakes were permitted to cool down before passengers disembarked onto the runway and were bussed to the terminal.

The Canadian TSB opened an investigation and dispatched investigators on site. The failure of an O-ring at the PTU manifold had caused a hydraulic leak, a fault was found in the PTU switch in the cockpit, which permitted the PTU to remain powered although the switch was selected off.
