An Antonov Design Bureau Antonov AN-124-100, registration UR-82007 performing flight ADB-5418 from Baku (Azerbaijan) to Leipzig (Germany), was enroute at FL300 when the #1 engine (D18T, outboard left hand) surged, a magnetic chip detector indication illuminated, rotor speeds dropped and the exhaust gas temperatures rose. The crew shut the engine down and continued the flight to Leipzig for a safe landing on three engines.

The Ukrainian Ministry of Transport reported in their monthly bulletin, that post flight inspection showed the engine cowling was damaged. Further examination revealed substantial damage to the low and medium pressure compressors, which seized the rotors. The engine was removed from the aircraft and underwent examination at the engine manufacturer, who established the first stage medium pressure compressor had been destroyed as result of a fatigue crack developing at the rim of the compressor disk. The crack was traced down to unusual loads during an engine test following engine overhaul in April 2003. The investigation recommended a prompt one time inspection of the first stage medium compressor for cracks and fatigue on all D18T engines.
