A Canjet Boeing 737-800, registration C-GDGT performing flight C6-770 from Toronto,ON (Canada) to Puerto Plata (Dominican Republic) with 189 people on board, had just reached cruise level 350 about 30 minutes into the flight when the crew noticed the outflow valve was unresponsive and the cabin pressure therefore not controllable. The crew performed a rapid descent to FL100 and returned to Toronto for a safe landing on Toronto's runway 33R about one hour later.

The Canadian TSB reported maintenance found the outflow valve covered with ice which prevented the operation of the valve. The ice came from a leaking water coupler from the potable water system, the leaking water running down into the valve and freezing in flight. According to a Boeing service bulletin the water tank has to be drained to prevent pipes freezing up if the aircraft is parked in extremely low temperatures. Due to the expansion of ice it will damage water couplers.
