A Motor Sich Airlines Yakovlev Yak-40, registration UR-87215 performing flight M9-6212 from Lviv to Kiev (Ukraine), was climbing through FL170 when the crew received indications of right hand engine vibrations. The crew shut the engine down and continued to Kiev for a safe landing.

Ukraine's Ministry of Transport reported in their monthly bulletin that the trailing edges of 6 blades of the first stage low pressure compressor were damaged with dents of up to 0.3mm on four blades. The investigation further established that the aircraft was climbing through a zone of severe icing, which resulted in ice building up at the first stage compressor blades resulting in an engine imbalance and vibrations.
