A Cargojet Airways Boeing 757-200, registration C-GCJT performing flight W8-919 from Calgary,AB (Canada) to Cincinnati,KY (USA), had just reached cruise FL350 when the crew observed the cabin altitude climb at 100fpm. Shortly afterwards the cabin altitude warning sounded, the crew donned their oxygen masks and performed an emergency descent. The aircraft returned to Calgary for a safe landing about 80 minutes after departure.

The Canadian TSB reported the Equipment Cooling Overboard Exhause Valve had been deferred and had been locked in the "SMOKE" position (partially open) according to minimum equipment list requirements prior to the flight. After the aircraft returned maintenance found the valve in the fully open position having failed in the locked out position. This created the loss of pressure. The valve was replaced.
