An Air Canada Rouge Boeing 767-300, registration C-FMWU performing flight RV-1836 from Phoenix,AZ (USA) to Toronto,ON (Canada) with 296 people on board, had landed on Toronto's runway 24L, vacated the runway via taxiway D6 and was instructed to hold short of runway 24R, which was correctly read back by the crew.

An Air Canada Rouge Airbus A319-100, registration C-GARJ performing flight RV-1876 from Toronto,ON (Canada) to Havana (Cuba), was cleared and was accelerating for takeoff from runway 24R, when the Boeing 767 crossed the hold short line 24R at D6 (at the end of runway 24R). ATC immediately instructed the Boeing to stop, the crew complied and stopped the aircraft short of the runway edge line.

The Canadian TSB reported the A319 was already airborne climbing through about 400 feet AGL when the aircraft crossed the intersection.
