Cavok AN12 at Hassi Messaoud on Apr 6th 2016, engine shut down in flight

A Cavok Air Antonov AN-12BP, registration UR-CCP performing freight flight from Marseille (France) to Hassi Messaoud (Algeria), was descending through FL100 towards Hassi Messaoud when engine #3 (AI-20, inboard right hand) developed increased vibrations, the oil pressure began to fluctuate and the oil quantity to decrease. The crew shut the engine down and continued for a safe landing at Hassi Messaoud.

Ukraine's National Bureau of Air Accident Investigations (NBAAI) reported that a post flight inspection of the engine revealed the #3 fairing was damaged, in addition the top and side cowl was missing and there was damage to the skin near frames 10 to 18. The occurrence was rated a serious incident and was delegated to the NBAAI by Algeria's Aviation Authority.

On Dec 15th 2016 the NBAII reported that the aircraft was descending through about 3000m towards Hassi Messaoud when the oil pressure of the #3 engine dropped sharply from 5.5kg/cm2 to 4.2kg/cm2, an oil leak developed in tank #3. At the same time the vibrations of the #3 engine began to increase from 1.9 to 4.0 units. The crew shut the engine down but could not feather the propeller, even via the emergency system. The crew continued for a safe landing. The NBAII analysed that the cause of the engine failure could not be determined due to the internal destruction of the engine. The engine cowls separated causing additional damage to the airframe when the vibrations of the engines exceeded the maximum limits due to the delayed shut down of the engine. The NBAII concluded the cause of the incident was the serious failure to shut the engine down after its failure. The cause of the engine failure could not be determined.

On Feb 3rd 2017 the NBAII released their final report in Ukrainian concluding the causes of the serious incident were:

- the failure to shut the #3 engine down after its failure

- the cause of the engine failure could not be determined due to the significant damage to the engine parts.
