A Jazz de Havilland Dash 8-300, registration C-GHTA performing flight QK-8657 from Toronto,ON to London,ON (Canada) with 54 people on board, was on approach to London descending through about 8000 feet when the crew heard a loud pop and observed a flash, flames and smoke from the first officer's window terminal block. The flame extinguished itself a short time later, the crew declared Mayday nonetheless. Radio contact with ATC was temporarily lost after the crew donned their smoke masks in combination with an incorrectly configured audio panel, the crew managed to restore communication after switching the radios. The aircraft landed safely in London.

The Canadian TSB reported maintenance replaced the first officer's windshield.
