An Endeavor Airlines Canadair CRJ-900 on behalf of Delta Airlines, registration N272PQ performing flight 9E-4070/DL-4070 from New York JFK,NY (USA) to Toronto,ON (Canada), landed on Toronto's runway 24L and was cleared to vacate the runway and hold short of runway 24R which was acknowledged by the crew. The aircraft vacated runway 24L via taxiway D4.

A LOT Polish Airlines Boeing 787-800, registration SP-LRB performing flight LO-42 from Toronto,ON (Canada) to Warsaw (Poland), was cleared for takeoff from runway 24R and was accelerating for takeoff, when the CRJ-900 crossed the hold short line, with red stop bar lights illuminated. Tower noticed the CRJ had crossed the hold short line and instructed the aircraft to stop immediately, the crew acknowledged and stopped the aircraft short of the runway edge line. Tower also cancelled the takeoff clearance for the B788, about 900 meters/3000 feet into their takeoff run by then, but received no reply. The takeoff was continued, the aircraft climbed out safely and continued to Warsaw for a safe landing.

The Canadian TSB reported the occurrence was rated a "risk of collision".
