Indonesia's NTSC released their final report concluding the causes of the crash were:

- The flight was in VFR however both pilots agreed to fly into the cloud, consequently, the flight crew had lack of situation awareness due to lost of visual references to the ground and no or late recovery action prior to impact due to low visibility.

- There was lack of good crew coordination due to steep cockpit transition gradient.

- There was no checklist reading and crew briefing.

The NTSC reported that the crew consisted of a captain (56, ATPL, 5,935 hours total, 3,730 hours on type) and a first officer (49, CPL, 2,500 hours total, 1,100 hours on type). The aircraft was enroute at 8000 feet about 35nm from Medan when the crew attempted to contact Kutacane three times however without reply from Kutacane. A discussion about the cloudy weather ensued between the pilots, there was no gap in the clouds to pass through. Both pilots agreed to fly into the clouds.

The NTSC reported based on cockpit voice recordings that the captain dominated the discussion, there was a strong gradient between the two pilots.

After flying into the clouds the crew discussed whether they had passed a certain point. About 4 minutes after flying into the clouds the cockpit voice recorder stopped working. The NTSC stated, that there was no discussion of any malfunction of the aircraft, no checklist reading whatsoever and the engine sound was constant. The aircraft did not carry a flight data recorder and was not required by law to do so.

The aircraft impacted Leuser Mountain at a slope of 70 degrees and came to rest at an altitude of 5,055 feet at position N3.4000 E98.0166. All occuptants were fatally injured by impact forces, the aircraft was destroyed.

When PK-TLF did not arrive in Kutacane and Kutacane began to inquire about the aircraft and Jakarta search and rescue offices reported receiving an ELT signal from the vicinity of the aircraft's path, two aircraft by Susi Air began a search for the aircraft, the crew of one aircraft located the wreckage 16nm from Kutacane and 40nm from Medan.

The NTSC analysed: "The conversation and impact area indicated that the pilots had lost the situational awareness."
