ITA A339 at Los Angeles on Oct 31st 2025, turned wrong on initial departure, loss of separation

An ITA Italia Trasporto Aereo Airbus A330-900, registration EI-HJO performing flight AZ-621 from Los Angeles,CA (USA) to Rome Fiumicino (Italy), had been cleared for takeoff from runway 24L RNAV to DLREY waypoint (about 1.5nm straight past the runway end), departed Los Angeles' runway 24L, but turned left during initial climb about 0.8nm past the runway end and came close to the departure path of Los Angeles' runway 25R. The aircraft had just been handed off the tower frequency to departure, when tower attempted to issue an instruction to the aircraft without success.

An American Airlines Airbus A321-200, registration N107NN performing flight AA-4 from Los Angeles,CA to New York JFK,NY (USA), departed Los Angeles' runway 25R and had just checked in with departure, when departure instructed the aircraft to stop the climb at 1500 feet, which the crew acknowledged.

Immediately afterward Departure instructed AZ-621 to fly a heading of 270 effectively turning the aircraft back onto runway heading. Departure subsequently queried why the aircraft turned south, they should have maintained runway heading, the crew responded "sorry". Departure told the crew a phone number to call for pilot deviation.

According to ADS-B data the separation between the aircraft reduced to 0 feet vertical and 0.6nm horizontal.

After being clear of conflict both aircraft continued to their destinations without further incident.
