A Jazz de Havilland Dash 8-400, registration C-GGNZ performing flight QK-8441 from Vancouver,BC to Terrace,BC (Canada) with 72 passengers and 4 crew, was climbing out of Vancouver's 08R when the crew reported an unusual vibration during the departure, suspected a blown tyre during the departure roll and requested a runway inspection. The runway inspection found tyre debris. A passenger advised it appeared the left hand inboard main tyre was damaged. The crew decided to return to Vancouver, positioned for an approach but went around. After positioning for another approach the aircraft landed safely on runway 08R about 95 minutes after departure.

The Canadian TSB reported: "On final approach, the company dispatch asked CYVR tower to advise the crew that a checklist for a main landing gear tire failure needed to be completed prior to landing. The crew initiated a missed approach, confirmed the checklist was completed, and landed without issue." and continued: "Maintenance found the left inboard main landing gear tire had blown and had caused minor damage to the inboard landing gear door and the lower panel of the engine nacelle. Maintenance replaced both tires, the inboard brake, and the inboard speed transducer on the left main landing gear. Temporary repairs were completed on the landing gear door and engine nacelle."
