An AerCaribe Antonov AN-32B, registration HK-4833 performing a flight from La Pedrera to Tarapaca (Colombia) with 40 passengers and 5 crew, veered right off the wet runway 25 while landing in Tarapaca in daylight at 17:12L (22:12Z). One passenger received minor injuries, the aircraft sustained substantial damage.

Colombia's Aerocivil reported the aircraft HK-4833 carrying 43 people suffered a runway excursion while landing in Tarapaca. There were no injuries.

Tarapaca offers a runway 07/25 with 1075 meters/3530 feet length.

In Aug 2018 Colombia's GRIAA released their final report in Spanish concluding the probable causes of the accident were:

- Improper decision by the operating company, when they abruptedly changed the destination of the aircraft to a destination unknown to the company, that was not suitable for the aircraft type and had not been authorized in the operator's operating specifications. The operator also did not perform any risk assessment.

- Inadequate decision by the crew to accept a destination, that was unknown to them, and proceeding there without due knowledge of the characteristics, without experience or training and without conducting at least a risk assessment.

- Runway excursion at 24 meters from the threshold runway 25 probably as result of an unstabilized approach that caused the landing outside the runway axis.

The following factors contributed to the accident:

- inefficient planning and supervision of aircraft operations by the operator, by scheduling an aircraft movement to an aerodrome unknown to the company

- Breach of the company's Manual of Dispatch and General Operations Manual, in relation to procedures that must be accomplished prior to operating new routes, new or special airports, in aspects such as analysis of route, risk management and crew requirements

- Unawareness of the runway in Tarapaca by the crew

The GRIAA reported the aircraft had been dispatched onto an unscheduled flight operations from Bogota to Aracuara to La Pedrera to Aracuara to Leticia (night stop) and further to Putumayo and return to Bogota. After landing in Aracuara the aircraft took 41 passengers (military personnel) on board to be taken to La Pedrera, however, the weather was unfavourable so that the crew diverted to Mitu. The aircraft refueled and subsequently was able to reach La Pedrera. At La Pedrera the passengers disembarked, another 40 passengers boarded the aircraft for the flight to Aracuara. While the crew prepared for the flight to Aracuara they received instruction by the company to take the aircraft to Tarapaca instead.

The crew departed for Tarapaca for a visual flight. While descending towards Tarapaca the crew noticed there were clouds near the threshold runway 07 estimated at 500 feet AGL. The crew decided to overfly the runway and subsequently decided to approach runway 07, however, positioned to and approached runway 25. After touchdown on runway 25 the aircraft exited the runway to the right before coming to a stop 438 meters past the runway threshold. The crew shut the engines down and initiated an evacuation of the aircraft. One passenger suffered minor injuries.

The aircraft sustained substantial damage including deformations at the right wing near the root and partial fracture of the nose gear.

The GRIAA analysed that the aircraft was properly prepared and within mass and balance for the flight from La Pedrera to Aracuara when a sudden change of flight plan occurred. The aircraft departed about one hour after the change of plans was communicated to the crew leaving them with one hour of preparation for the flight.

After overflying the runway the crew set the flaps for 38 degrees, decided to land on runway 07, however performed the final approach to runway 25 at 230 kp/h (118 KIAS). No abnormal communications occurred in the cockpit, no abnormal sound was recorded. It was evident however that the right main gear left the right edge of the runway just after touchdown, 24 meters past the threshold of the runway. This immediate runway excursion probably occurred because the aircraft was not aligned with the runway center line as result of an unstabilized approach. This probably was also the result of not being familiar with the properties of the runway which permitted only 3.05 meters of maneouverability each side of the runway as well as the lack of assistance to the crew such as a wind sock. Although the crew had decided to land on runway 07, in reality they performed the approach to runway 25. The GRIAA stated: "This lack of situational awareness of the crew can be directly related to the obvious lack of knowledge of the aerodrome's operational environment."
