A LOT Polish Airlines de Havilland Dash 8-400, registration SP-EQC performing flight LO-654 from Cluj (Romania) to Warsaw (Poland), was climbing through 2950 feet out of Cluj about a minute after becoming airborne when the crew received a master caution door indication. The crew continued the climb to FL080 to observe the cabin pressure and detected that the cabin was not properly pressurizing. The aircraft therefore returned to Cluj for a safe landing.

The aircraft remained on the ground for about an hour, then departed again and reached Warsaw with a delay of about an hour.

Poland's PKWBL released their final report in Polish concluding the probable cause of the incident was:

the incorrect closure of the door of the aft cargo compartment.

The PKBWL reported the door was opened and closed after landing, which removed a gap. The PKBWL recommended a review of Cluj Airport's ground handling's divison.
