A United Boeing 787-900, registration N27958 performing flight UA-128 from Rio de Janeiro Galeao,RJ (Brazil) to Houston,TX (USA) with 221 people on board, was descending towards Houston when the crew deviated around weather depicted on their weather radar, the fasten seat belt signs were illuminated and the crew was instructed to complete service and be seated for arrival. After being clear of such weather the crew intended to return on course but encountered moderate, then a few moments severe turbulence causing a flight attendant in the aft galley to be lifted off the floor and falling back onto the floor resulting in the fracture of her ankle. The crew continued for a safe landing at Houston's Intercontinental Airport, the injured flight attendant was taken to a hospital.

On Aug 25th 2016 the NTSB released their brief final report concluding the probable cause of the accident was:

An inadvertent encounter with convective turbulence resulting in serious injury to a flight attendant
