A Safair Boeing 737-400, registration ZS-JRE performing flight FA-103 (callsign SFR-901) from Johannesburg to Cape Town (South Africa) with 155 passengers, was enroute at FL320 about 120nm southwest of Johannesburg when the crew initiated a rapid descent and released the passenger oxygen masks due to the loss of cabin pressure. The aircraft returned to Johannesburg for a safe landing.

The airline reported the aircraft again suffered a gradual loss of cabin pressure, similiar to the event two days earlier, see Incident: Safair B734 near Johannesburg on Jan 22nd 2016, loss of cabin pressure. The aircraft had just reached FL320 when the crew noticed the cabin pressure did not stabilize, manually released the passenger oxygen masks and performed the according safety procedures.

A replacement Boeing 737-400 registration ZS-JRI reached Cape Town with a delay of 3.5 hours.

On Jan 27th 2016 the airline reported that the causes as well as the encounter on Jan 22nd 2016 have been fully identified. The airline reported that after the occurrence on Jan 22nd 2016 maintenance found a loose clamp on a duct, repaired the clamp, performed pressurization tests on the ground followed by a flight test up to FL360 with no further findings, then returned the aircraft to service. During the second event on Jan 24th 2016 a cargo door seal caused the gradual loss of cabin pressure.
