A LOT Polish Airlines Boeing 787-800, registration SP-LRA performing flight LO-41 from Warsaw (Poland) to Toronto,ON (Canada), was descending through about 9000 feet towards Toronto when approach queried the crew to use another transponder, they had lost their tag on the radar screen and were not receiving any signal from their transponder. Approach queried again a minute later, the crew responded they had tried the other transponder but received a fail indication. Approach continued normal service for flight LO-41 who continued for a safe landing on Toronto's runway 05 about 12 minutes later.

NAV Canada did not include that occurrence in their daily reports.

The aircraft departed Toronto for the return flight LO-42 with a delay of 3.5 hours but needed to remain at FL270 below RVSM airspace and subsequently diverted to Keflavik (Iceland) due to the transponder problem and Norway refusing overflight permission, see Incident: LOT B788 near Toronto on Sep 28th 2013, technical problems.
