A Smartwings Bombardier C-Series CS-300, registration OK-EYA performing flight QS-1019 from Cagliari (Italy) to Prague (Czech Republic), was preparing for departure when ground crew discovered damage to the aft main door L2 and reported the damage to the captain, who inspected the door and could not find any damage. The aircraft departed to Prague and landed without further incident. The report of door damage was not recorded in the tech log nor was it communicated to maintenance.

The aircraft departed for the next rotation to Valencia,SP (Spain) as flight QS-1054. After landing in Valencia, while preparing for the return flight QS-1055, a belt loader collided with the frame of the front cargo door, the damage was reported to the captain, however, the captain again did not report the damage to maintenance or recorded it in the techlog. The aircraft departed for flight QS-1055 and landed in Prague without further incident.

Czechia's UZPLN reported the damage to the cargo door was only recorded in the tech log after landing in Prague. Ground personnel found damage (scratches, dents) in areas where no damage is permitted and grounded the aircraft. The aircraft manufacturer was involved. Only then the damage to the door L2 was discovered too.

The UZPLN stated it were bad decisions by the captain to not report the damage. The operator took corrective action by interviews with the captain regarding compliance with regulations and procedures in case of aircraft damage, and issued a safety bulletin.
