A Summit Air Avions de Transport Regional ATR-72-200, registration C-GUSA performing flight SMM-7212 from Fort Good Hope,NT to Deline,NT (Canada) with 3 crew, was in the initial climb through 500 feet AGL out of Fort Good Hope when the crew observed the cabin was not pressurizing as would be normal, a cargo door unlocked light illuminated. The load master went back to inspect the cargo door, in that process the cargo door became unlatched and opened in flight. The load master returned to the cockpit and the crew decided to divert to Norman Wells,NT (Canada) where the aircraft landed without further incident.

The Canadian TSB reported the indoor cargo door lining was torn from the cargo door by the slip stream and struck a propeller blade and the leading edge of the left center wing section. Maintenance found no issue with the door latching mechanism, the door had been improperly latched on the ground prior to departure.
