A Kalitta Air Boeing 747-400, registration N402KZ performing flight K4-652 from Anchorage,AK to Miami,FL (USA), was enroute at FL330 about one hour into the flight when the crew observed that the #1 engine (CF6, left outboard) was being fed from the #1 main fuel tank rather than the inboard tanks as selected, the #1 crossfeed valve appeared to be in the closed position with the crew being unable to select it into the open position. As this was insufficient to supply fuel to the engine for the remainder of the flight, the crew consulted with the company operations center. The engine was shut down, the aircraft descended to FL290, the decision was made to continue the flight to destination. When the fuel remaining in the #1 main fuel tank was sufficient to supply the engine for the rest of the flight, the crew performed an inflight re-start of the engine and continued the flight without further incident for a safe landing in Miami.

The Canadian TSB reported: "Company maintenance subsequently replaced the #1 engine crossfeed valve actuator."
