A Cargojet Airways Boeing 767-300 freighter, registration C-GAZF performing flight W8-2384 from Vancouver,BC to Hamilton,ON (Canada) with 2 crew, was climbing out of Vancouver at about 10,000 feet when the crew received an "AUTOPILOT" EICAS advisory. The crew stopped the climb to work the related checklists, the advisory extinguished and the crew continued the climb. At about 16000 feet the advisory re-appeared and the crew noticed the autopilot and flight director began to disagree, the airspeed was decreasing. The crew disconnected the right hand autopilot and the aircraft abruptedly pitched up due to being significantly out of trim. The crew declared PAN PAN and initiated a return to Vancouver. During the descent a "STAB TRIM" EICAS advisory occurred. The aircraft landed safely back in Vancouver with emergency services on stand by.

The Canadian TSB reported the aircraft had been dispatched with the left hand and center autopilots inoperative due to earlier defects. The right hand autopilot was also deferred, the aircraft refuelled and the aircraft flown to Hamilton manually at FL270. During that flight the "STAB TRIM" advisory came up twice. Maintenance replaced left hand and right hand stabilizer trim limit switches and replaced the left hand stabilizer actuator motor to adress the recurring trim issues. The autopilots were tested to perform properly.
