A Cargojet Airways Boeing 757-200 freighter, registration C-FLAJ performing flight W8-9001 from Regina,SK to Hamilton,ON (Canada) with 2 crew, was enroute at FL410 about 80nm south of Winnipeg,MB (Canada) in US airspace when the crew heard a noise from the upper cargo deck followed by an EICAS cabin altitude warning and a master warning. The crew initiated an emergency descent, worked the related checklists and diverted to Winnipeg for a safe landing on runway 18 about 35 minutes after leaving FL410.

The Canadian TSB reported maintenance found a fault with the cabin pressure outflow valve. The aircraft was released under minimum equipment list requirements and continue the flight to Hamilton unpressurized. The outflow valve was susbequently replaced.
