A Cargolux Boeing 747-400 freighter, registration LX-TCV performing flight CV-7756 from Calgary,AB (Canada) to Luxembourg (Luxembourg) with 3 crew, was enroute at FL310 about 20nm west of Churchill,MB (Canada) when the crew needed to shut the engine #4 (RB211) down. The aircraft turned around, descended to FL300 and returned to Calgary for a safe landing on runway 35L about 2:40 hours after turning around. Emergency services inspected the aircraft, the crew advised they had severe engine #4 damage, the aircraft subsequently taxied to the apron.

On Apr 19th 2021 the Canadian TSB reported about 100 minutes into the flight the crew heard a loud bang associated with vibrations and N1 fluctuations by the #4 engine (outboard right hand). The crew reduced the thrust lever to idle, the engine parameters stabilized initially, however, the EGT began to rise and was finally displayed in a red box. The crew thus performed the severe engine damage checklist and shut the engine down. The crew declared PAN PAN and initiated a diversion to Winnipeg,MB (Canada) and coordinated with ATC to dump fuel. As the situation was under control and after consultation with dispatch the crew decided to return to Calgary, declared Mayday and performed a safe landing in Calgary. Maintenance replaced the #4 engine, that had produced vibration indications in the two legs prior to the occurrence flight with no anomalies found by maintenance following both flights.
