A British Airways Boeing 787-9, registration G-ZBKD performing flight BA-293 from London Heathrow,EN (UK) to Washington Dulles,DC (USA), was enroute at FL410 about 190nm northeast of St. John's,NL (Canada) when the left hand air conditioning pack failed prompting a "PACK ALTITUDE LIMIT" warning. The crew requested to descent to FL340 as required by the checklists, the descent however was denied by ATC. The crew declared PAN PAN due to a pressurization issue and was cleared to FL340. Once at FL340 the crew reported the situation was normal and cancelled the PAN. The aircraft continued to destination without further incident.

The Canadian TSB reported the aircraft had been dispatched with the right hand air conditioning system inoperative under minimum equipment list requirements. When the left hand air conditioning pack failed at FL410, the aircraft was required to descend to FL340 indicated also by the PACK ALTITUDE LIMIT message.
