A Lion Air Boeing 737-900, registration PK-LFK performing flight JT-600 from Jakarta to Jambi (Indonesia) with 206 passengers and 7 crew, was climbing out of Jakarta when the flight crew noticed the cabin was climbing at an excessive speed and noticed the cabin pressure control selector was in manual position. The pilot selected the outflow valve selector to open (editorial note: ??? The NTSC repeated this statement 3 times throughout the report) and continued the climb. At 10,000 feet the cabin altitude warning activated, the crew decided to level off at 10,000 feet for a while and pressed the ALT HORN cutout switch, the cabin altitude warning ceased. The aircraft subsequently continued the climb to 18,000 feet when flight attendants informed the flight crew that the passenger oxygen masks had been released. The crew requested to divert to Palembang advising of a pressurization problem. The aircraft descended to 15,000 feet and landed safely in Palembang about 40 minutes after the decision to divert. The aircraft taxied to the stand, at the stand the crew noticed a cabin pressure differential of 5.8 psi, the cabin altitude indicated negative. The crew discussed this with a ground engineer who recommended to manually open the outflow valve. After the outflow valve opened manually, the pressure differential disappeared and the cabin doors could be opened.

Indonesia's NTSC released their preliminary report stating that the flight crew operating the aircraft on the previous sector the previous day had reported that the cabin rate of descent was hard to control during the descent. The NTSC described the subsequent maintenance actions:

The engineer performed bite test on Cabin Pressure Controller (CPC) number 1 and CPC number 2, the result of bite test found that LRU (Line Replacement Unit) of CPC number 1 failed. It was written in the AFML that the LRU CPC number 2 failed.

The engineer requested to Maintenance Control Center for a serviceable CPC to replace the fail CPC and was informed that the serviceable CPC was not available.

The engineer reviewed and discussed the situation with chief of the shift and advised to issue the hold item into Deferred Maintenance Item (DMI), refer to B737 Series Minimum Equipment List (MEL) Chapter 21-14(3). The engineer issued a DMI No. 022796.

As result the aircraft departed with a single cabin pressure controller (CPC) working.
