SA Express CRJ2 near Johannesburg on May 16th 2018, engine rolled back in flight

A South African Express Canadair CRJ-200, registration ZS-NMI performing flight SA-1123 from Johannesburg to Mmabatho (South Africa) with 19 passengers and 4 crew, was climbing through 8000 feet out of Johannesburg when the crew observed the aircraft yaw to the left unexpectedly and noticed the left hand engine (CF34) was rolling back. The crew declared PAN PAN PAN, entered a hold at 9000 feet MSL to work the checklists and prepare the aircraft for the return, then returned to Johannesburg for a safe single engine landing on runway 21L about 34 minutes after departure.

South Africa's CAA (SACAA) reported a post flight inspection did not reveal any engine damage or obvious reason for the roll back. The fault finding checklist recommended to check the drive shaft between Fuel Pump and Fuel Metering Unit (FMU) for continuity. The FMU was removed and the drive shaft was found to show excessive wear at the FMU side, the gear train also showed excessive wear causing the drive gear to slip in operation which disrupted the fuel supply to the engine and resuled in the engine shut down. The engine was a "loaner engine", which had been installed on the aircraft on Jul 14th 2017 and had accumulated 40,904 hours and 35,877 cycles since new. The drive shaft as well as the FMU were replaced.

On Aug 24th 2018 the SACAA released their preliminary report basically re-iterating the information already provided earlier and adding photos of the FMU and drive shaft.
