Bukovyna MD88 near Mashad on Jul 24th 2012, cabin pressure problems

A Bukovyna McDonnell Douglas MD-88 on behalf of Taban Air, registration UR-CIX performing flight HH-6261 from Mashad to Kish Island (Iran), was climbing through FL220 out of Mashad when the crew observed the cabin altitude climb through 8000 feet. Although both air conditioning systems showed no malfunction the cabin altitude continued to climb at 200 feet per minute. At FL270 the commander stopped the climb and decided to return to Mashad. During the descent, at about FL190, the cabin pressure returned to normal. At FL160 the commander decided to stop the descent and continue the flight. While climbing through FL270 the cabin altitude again climbed through 8000 feet prompting the captain to decide to return to Mashad, where the aircraft landed safely.

Ukraine's Ministry of Transport reported in their monthly bulletin that maintenance found a defective flow control valve, a faulty pressure regulator and clogged filters of both left and right hand air conditioning systems. The filters and the faulty devices were replaced. The investigation commission analysed that the faults were caused by a manufacturing fault, the manufacturer disagreeing with that finding. The commission thus stated in their conclusions that the reasons for the failure of the devices could not be established.
