A Trans States Airlines Embraer ERJ-145 on behalf of American Airlines, registration N621AE performing flight AX-4216/AA-4216 from New York La Guardia,NY (USA) to Toronto,ON (Canada) with 53 people on board, was on approach to Toronto when a loud thud was heard in the cabin followed by smoke entering the cabin. The flight crew donned their oxygen masks and continued for a safe landing on Toronto's runway 06L, vacated the runway via high speed turn off C1 and stopped past the hold short line. Cabin crew advised the smoke was continuing, the captain initiated an emergency evacuation through the main cabin door onto the taxiway.

The return flight, still AX-4216, was cancelled.

The Canadian TSB reported maintenance found the bleed air duct under the cabin had separated. The ducts were reconnected and the aircraft returned to service.
