On Mar 30th 2017 the Indonesian NTSC reported the aircraft blew both nose tyres on landing, the wheel hubs scratched the runway surface. The aircraft came to a stop on the intersection with taxiway A, the runway needed to be closed. Damage was found to the steering linkage and the nosewheel axles were bent. The occurrence was rated an accident and is being investigated by the NTSC.
On Aug 18th 2017 Indonesia's NTSC released their preliminary report reporting the captain (55, ATPL, 13,769 hours total, 645 hours on type) was pilot flying, the first officer (30, CPL, 1,648 hours total, 1,403 hours on type) was pilot monitoring.
Descending through 1500 feet the flight was cleared to land and was advised the winds were from 270 degrees at 4 knots. At 500 feet AGL the autopilot was disconnected, the yaw damper was disconnected at 200 feet AGL. The crew subsequently requested towing assistance. While waiting for the tow Halim Tower informed the crew, that the nose gear was damaged and instructed the crew to shut the engines down, recommending the passengers should disembark onto the runway.
The NTSC stated the aircraft received substantial damage: both nose tyres blown, the nose wheel hubs scraped the runway and the nose wheel axle was bent. In a subsequent examination cracks were found in the mounting strut bar and the shock mount of engines #1 and #2. The runway received scrape damage about 5cm deep along the aircraft movement.
