Poland's PKBWL reported that at the time of the uncommanded climb the flight mode announciators indicated LNAV and VNAV PTH rather than LNAV and ALT. The MCP also showed LNAV and VNAV activated. Unaware of the altitude contraint imposed by ATC and being programmed to FL380 prior to departure the FMC thus initiated a shallow climb at about 100-200fpm. The PKBWL stated that insufficient supervision of the programming of the FMC prior to departure by just one pilot as well as insufficient supervision of flight parameters in flight caused the occurrence. The PKBWL reported the airline took a number of safety actions as result of the occurrence.
