The Canadian TSB reported the aircraft had been dispatched with the left hand pressure regulating shut off valve locked in the close position under minimum equipment list requirements. Maintenance found a clamp loose on the right hand precooler, the resulting leak caused the right hand pressure regulating valve to also close resulting in the loss of cabin pressure.
NAV Canada reported no emergency was declared.
