Spain's CIAIAC opened an investigation stating:
On Wednesday, September 17, 2025, an improper approach occurred between the Boeing B737-800 aircraft (aircraft 1), registration EI-DWX, and the Airbus Helicopters EC135 P3H aircraft (aircraft 2) at the Madrid TMA.
Aircraft 1 had taken off from runway 36L at LEMD. Aircraft 2 was traveling parallel to and east of runway 36R. The crossing between the two aircraft occurred when aircraft 1 was in its initial climb and aircraft 2 was to the right of the runway 36R extension, while each was controlled by a different air traffic control unit and communicating on different radio frequencies. The closest approach between the two aircraft was 1.4 NM horizontally and 500 ft vertically.
No evasive maneuver was performed by either aircraft.
There were no injuries, nor any damage to the aircraft.
