A Ukraine International Airlines Antonov AN-148, registration UR-NTD performing flight PS-24 from Kharkov to Kiev (Ukraine), was climbing out of Kharkov about 2 minutes into the flight when the crew received a low fuel pressure warning for the right hand engine (D436) and indication that both fuel pumps on the right hand fuel system had failed suggesting the engine was gravity fed with fuel. After working the checklists to prevent air entering the fuel lines the low fuel pressure indication ceased, the failed fuel pump indications continued. The crew levelled off at FL200, the maximum altitude permitted with gravity fuel feeding. The captain subsequently tried to re-engage each right hand fuel pump without success, instead the fuel minimum pressure indication re-illuminated prompting the crew to decide to return to Kharkov, where the aircraft landed safely.

Ukraine's Ministry of Transport reported in their monthly bulletin that both wires supplying the fuel pumps with electrical energy were found powerless due to a damaged wiring harness near the rear spar at rib 17. It was concluded an electrical short due to damage of the insulation as result of lack of mechanical protection of the harness against external influences caused an eletrothermal fracture of the wires.
