An Aeroservice LLC Let L-410 on behalf of Sila Avia, registration RA-67042 performing flight 51 from Irkutsk to Kazachinsk (Russia) with 14 passengers and 2 crew, was on approach to Kazachinsk at 22:35L (14:35Z) when the aircraft impacted trees at the cliff of an island in the middle of the Kirenga River about 4000 meters/2.16nm southwest of the runway. The first officer and three passengers were killed, 4 passengers were able to free themselves, 8 passengers were freed from the wreckage by emergency services. All survivors were taken to hospitals with moderate to serious injuries. The aircraft was destroyed.

Sila Avia reported the aircraft was on its second approach in heavy fog when it disappeared from radar.

Russia's Ministry of Emergency Situations reported emergency services are moving to the accident location of a L410 suffering a hard landing. 14 passengers and 2 crew were on board.

According to Russia's regional investigative committee a passenger first alerted emergency services to the accident, all passengers were rescued by emergency services. According to passenger reports one of the pilots was killed in the accident.

The governor of the Irkutsk region reported all occupants of the aircraft survived, there had been no fatality.

According to local media emergency services have reached the accident location in the Taiga, hard to reach, at about Sep 13th 01:30L (Sep 12th 17:30Z). Until that time emergency services were talking to the passengers via phones. The accident location is reported on an island (editorial note: which suggests the northern final position).

Subsequently local media, now also quoted by Russian News Agencies, reported that emergency services after reaching the accident site found 4 people dead, amongst them the captain of the flight, the first officer is said to have survived and to be in a state of shock.

Around 18:55Z the governor of Irkutsk reported the pilot as well as three passengers have been killed in the accident.

At about 19:00Z Russia's Ministry of Emergency Situations (MCHS) confirmed 4 fatalities were recovered, works are still ongoing to free trapped people.

At 19:45z Irkutsk's governor reported in addition to the 4 bodies removed from the airframe 8 survivors were recovered with moderate to serious injuries and handed over to medics. 4 other survivors have been able to free themselves and are being taken care of by medics, too.

On Sep 13th 2021 the East Siberian Transport Prosecution Office reported they are looking into the accident. One of the black boxes was recovered.

On Sep 13th 2021 Kazachinsk Airport stated, the first officer of the flight was killed, the captain of the flight survived. The crew had reported problems with their navigational systems.

On Sep 13th 2021 the Interstate Aviation Committee (MAK) reported they have opened an investigation into the accident, which killed one crew member and 3 passengers when the aircraft suffered an accident in the vicinity of Kazachinsk.

On Sep 14th 2021 the East Siberian Transport Prosecution Office reported both black boxes have been recovered. The accident site is about 4000 meters southwest of the airport.

Kazachinsk Airport (Position N56.2775 E107.5645) features a paved runway 04/22 of 1600 meters/5250 feet length.

The Interstate Aviation Committee (aka MAK) released their final report in Russian only (Editorial note: to serve the purpose of global prevention of the repeat of causes leading to an occurrence an additional timely release of all occurrence reports in the only world spanning aviation language English would be necessary, a Russian only release does not achieve this purpose as set by ICAO annex 13 and just forces many aviators to waste much more time and effort each in trying to understand the circumstances leading to the occurrence. Aviators operating internationally are required to read/speak English besides their local language, investigators need to be able to read/write/speak English to communicate with their counterparts all around the globe).

The report concludes the probable causes of the accident were:

The cause of the accident with the aircraft L 410 UVP-E20 RA-67042 was the crew's failure to comply with visual flight rules at night, which resulted in performing an approach when visibility is below the established minimum values, which led to a collision with natural obstacles in a controlled flight.

The contributing factors in an aviation accident are most likely appeared:

- discrepancy between the coordinates of the runway ends of the Kazachinskoye settlement in the GPS receivers of the PIC and co-pilot to their actual values, which led to incorrect calculations on landing;

- failure of the PIC to make a timely decision to leave for an alternate airfield if available he has information about weather conditions not meeting the established minimum values. The individual psychological characteristics of the PIC’s personality made it possible for him to make leadership decisions, but if they were wrong, he did not have abilities to correct them and was prone to unreasonably risky, dangerous decisions;

- use of the autopilot by the crew during the final stage of the flight, which is not complied with the provisions of the RLE. Selected autopilot operating modes significantly reduced the crew's situational awareness. In fact, the decrease was carried out significantly to the right of the extended axis of the runway over the terrain in which there were no light landmarks;

- lack of proper interaction among the crew and insufficient control over flight parameters.
