Wasaya DH8A at Pickle Lake on Oct 22nd 2019, WOW causes runway excursion

A Wasaya Airways de Havilland Dash 8-100, registration C-FABA performing flight WSG-815 from Pickle Lake,ON to Fort Severn,ON (Canada) with 2 crew only, was descending towards Fort Severn when the crew received a WOW (weight on wheels) caution light and decided to return to Pickle Lake. While on the return to caution light extinguished but came on again on approach to Pickle Lake. The aircraft landed on Pickle Lake's runway 09, the crew received several caution lights on touch down, the aircraft did not respond to steering tiller inputs, below 30 knots the aircraft brakes were unresponsive, the crew lost directional control and the aircraft veered off the runway coming to a stop about 100 feet north of the runway. There were no injuries, the aircraft sustained minor damage.

The Canadian TSB reported an examination of the aircraft revealed two faulty proximity sensors. No investigation has been opened.

On Jan 23rd 2020 the TSB released an Aviation Safety Information Letter indicating that the emergency procedures for the weight on wheel lights did not provide for the failure of both proximity sensors and the systems lost as result of both sensors failing, but provided only for the failure of one sensor. The TSB wrote:

The current emergency procedure listed in the AFM/QRH do not address the multiple system failures that can occur if the source of the “WT ON WHEELS” caution light is the failure of two proximity sensors rather than just one; therefore, following the current emergency procedure may lead to similar occurrences.

This occurrence demonstrates the need to provide specific information in the AFM/QRH “WT ON WHEELS” (Caution light) procedure regarding the possibility that the aircraft may not respond to anti-skid brakes and nose wheel steering input if two proximity sensors have failed. Therefore, Transport Canada (TC) is encouraged to work with the manufacturer to amend the “WT ON WHEELS” emergency procedure currently contained in the AFM and QRH.
