Peace B735 at Lagos on Nov 5th 2019, engine shut down in flight

An Air Peace Boeing 737-500, registration 5N-BUJ performing flight P4-7004 from Lagos to Owerri (Nigeria) with 84 passengers and 6 crew, was climbing out of Lagos when the crew needed to shut an engine (CFM56) down. The aircraft returned to Lagos for a safe landing about 31 minutes after departure.

Nigeria's Air Accident Investigation Board reported they opened an investigation into the occurrence.

On Feb 4th 2020 Nigeria's AIB released their preliminary report reporting the first officer (33, ATPL, 3,353 hours total, 3,047 hours on type) was pilot flying, the captain (42, ATPL, 8,900 hours total, 4,000 hours on type) was pilot monitoring.

The aircraft had been cleared to climb to FL310 out of Lagos and was climbing through FL230 when a loud bang occurred, the automation disengaged and the aircraft veered a little bit to the right. A second bang occurred a few seconds later. A glance at the engine instruments showed N2 of the right hand engine was at zero and N1 was running down through 30%. The crew worked the severe engine damage checklist and carried out the memory items. The captain took control of the aircraft and had the APU started. The captain decided to return to Lagos and requested runway 18R for landing. ATC advised them to expect a RNAV or VOR/DME approach to runway 18R, the crew advised: "WE HAVE AN EMERGENCY WE CAN NOT DO A VOR APPROACH WE HAVE AN EMERGENCY WE LOST AN ENGINE WE CAN’T DO A VOR APPROACH". The aircraft landed without further incident on Lagos' runway 18R 31 minutes after departure.

The occurrence engine had been removed from another Air Peace Aircraft on June 22nd 2018 and was subsequently repaired. On Apr 11th 2019 an Authorized Release Certificate was issued for the engine, which was installed on 5N-BUJ on Apr 17th 2019 with a TSN 83,611 hours and CSN of 76,567 cycles. 5N-BUJ performed the first flight with the newly installed right hand engine on Jun 17th 2019. On Oct 19th 2019 the right hand engine's EGT indicated erratically, the #2 engine EGT harness was replaced on Oct 20th 2019. On Nov 5th 2019 (the incident flight) at TSN 84,197 and CSN 77,222 respective time since repair of 586 hours and 655 cycles since repair the N1 and N2 gauges showed below normal values at FL230 enroute Lagos to Owerri.
