During examination of the aircraft the AAIB also noted that the right hand feather lever was not in the engaged (pulled) position, the cause being that the locking mechanism had not locked the lever in its active position due to a worn bore, consequently the right hand propeller had not reached its feathered position although both levers had been operated.
The daily checks before the first flight of the day did not catch the fact, that the feather lever would not lock in its active position, as the pilots believed the primary purpose was to make sure the fuel and hydraulic cocks closed, but were not aware that the detent should also have been checked. The AAIB therefore released a safety recommendation.
In addition to the safety recommendation already released with special bulletin another safety recommentation was issued to introduce a regular inspection to detect corrosion in the bores of the yoke pintles.
Another safety recommendation concerned the airport's communication between tower and emergency services, the reason being that when tower pressed the crash button there was no confirmation that the alert had been activated, the controller then called emergency services via the direct phone line, however emergency services did not pick up the phone as they were already deploying, the controller terminated the call when he saw the doors of the fire station open.
