An Air Canada Rouge Boeing 767-300, registration C-FMWV performing flight RV-1940 from Vancouver,BC to Dublin (Ireland) with 257 passengers and 8 crew, was climbing through 10,000 feet out of Vancouver a loud bang was heard from the right hand engine (PW-4060), the engine lost power and abnormal engine indications occurred. The crew reduced the engine to idle, declared emergency and returned to Vancouver for a safe landing on Vancouver's runway 08R about 25 minutes later. A runway inspection discovered debris on the runway, the runway was closed.

The Canadian TSB rated the occurrence an accident but did not open an investigation. The TSB reported: "An initial visual inspection of the engine revealed extensive damage to the aft-most turbine blades. The inboard aileron and the fuselage just aft of the baggage door were punctured by debris exiting the engine exhaust duct. The engine was sent to the manufacturer for teardown and inspection. The damage was limited to the Low Pressure Turbine (LPT) section, from the stage 4 vanes to the stage 6 turbine blades. Root cause analysis is on-going."
