LC Peru DH8D at Ayacucho and Lima on Aug 19th 2018, could not extend nose gear

A LC Peru de Havilland Dash 8-400, registration N404AV performing W4-1323 from Lima to Ayacucho (Peru) with 59 passengers and 5 crew, was on approach to Ayscucho when the crew received an unsafe indication for the nose gear. The crew went around, worked the related checklists and decided to return to Lima. The aircraft performed a low approach to Lima's runway 15 which confirmed the nose gear was not down. The aircraft subsequently positioned for another approach to runway 15 and landed about 2:48 hours after departure, the crew held the nose up as long as practicable before lowering the nose onto the runway causing sparks and smoke from the nose. The aircraft came to a stop without further incident, the passengers evacuated the aircraft. There were no injuries.

The airport confirmed an aircraft of LC Peru made an emergency landing.

Peru's National Emergency Operations Center reported the LC Peru Aircraft landed in Lima due to problems with the landing gear. No injuries occurred.

Peru's Comisión de Investigación de Accidentes de Aviación (CIAA) wrote in their preliminary declaration reporting that there had been a gear unsafe condition on the previous flight already. A mechanics was called and worked on the issue, however, no log book entries were made with respect to the unsafe condition and the corrective action.

The aircraft departed for flight W4-1323 and was on approach to Ayacucho when the crew selected the gear down, received green indications for both main gear, the nose gear indication remained red however. The crew also checked the alternate gear indication. Due to the lack of maintenance facilities the crew decided to return to Lima.

On approach to Lima the crew attempted another normal gear extension, however, with the same result (nose gear indicating red). The crew therefore attempted an alternate gear extension, then requested a low approach to have the gear inspected from the ground. Tower reported the nose gear doors were open, the nose gear could not be seen however.

The crew thus decided to land nose gear up. After evacuation of the passengers maintenance arrived at the aircraft to assess the damage and find ways to remove the aircraft from the runway. Maintenance engineers manually pulled the alternate gear extension lever located at the cabin floor and managed to extend the nose gear. The aicraft was then towed to a maintenance hangar.
