Far Eastern MD82 at Taipei on Jul 2nd 2018, uncontained engine failure

A Far Eastern Air Transport McDonnell Douglas MD-82, registration B-28035 performing flight FE-8026 from Makung to Taipei Songshan (Taiwan) with 165 people on board, was on final approach to Songshan's runway 28 descending through 2800 feet about 7nm before touchdown, when the left hand engine (JT8D) failed prompting the crew to shut the engine down and declare "PAN PAN". The aircraft continued for a safe landing on runway 28 about 3 minutes later.

Taiwan's ASC reported as result of internal damage of the left hand engine the cowling of the engine was pierced. The ASC rated the occurrence an accident and dispatched a team of investigators on site, flight data and cockpit voice recorder have been secured, the engine is being examined. The ASC stated: "Further information related to the occurrence will be published after the completion of the investigation."

On Dec 6th 2019 Taiwan's TTSB (former ASC) released their final report in Chinese only as well as an executive summary in English (Editorial note: to serve the purpose of global prevention of the repeat of causes leading to an occurrence an additional timely release of all occurrence reports in the only world spanning aviation language English would be necessary, a Chinese only release - even with a brief executive summary in English - does not achieve this purpose as set by ICAO annex 13 and just forces many aviators to waste much more time and effort each in trying to understand the circumstances leading to the occurrence. Aviators operating internationally are required to read/speak English besides their local language, investigators need to be able to read/write/speak English to communicate with their counterparts all around the globe).

The final report concludes the causes of the accident were (taken from English executive summary):

One of the 58 Low Pressure Turbine 4th stage (LPT4) blades on left engine of occurrence aircraft was fractured due to high cycle fatigue, the fracture occurred before its next scheduled check time is due, which was after performed an Airworthiness Directive (AD) check per CAA-2011-03-013 requested, the blade did not reach its designed initial check schedule when the blade fractured either. The fractured blade caused the fall-off of 5 segments of the LPT4 stator blades, the displacement of No. 6 bearing and the fracture of the other 57 LPT4 blades, and resulted in an uncontained engine failure by the fractured blades penetrated through the turbine fan bypass case and engine cowling.

Findings related to risk:

In the past 3 times of LPT4 shroud notch wearing checks, there were twice that FAT maintenance personnel did not in compliance with AD CAA-2011-03-013 requested to be performed with engine on-wing condition.

There was no manufacturing serial number of LPT4 blade on the occurrence engine for maintenance control, and no traceable historical records with total time usage since new either.
